Wisconsin Vehicle Power of Attorney Form

The Wisconsin vehicle power of attorney form is a document whereas the Principal would provide indefinite powers to an Agent to act on behalf of the Principal with regard to all matters pertaining to sales, transference of ownership or any other required transaction regarding any type of vehicle to be purchased or sold by the Principal and/or their business. The document must be witnessed by a notary. This document may be revoked, in writing, by the Principal at any time.

How to Write

Step 1 – The Parties – Enter:

  • The name of the individual or company recognized as the Principal
  • Name of the Agent
  • Signature of authorized company officer or individual

Step 2 – Additional Required Information –

  • If the powers are in the name of an individual enter their date of birth in mm/dd/yyyy format
  • Social Security Number (SSN)
  • If the powers are in the name of a company enter the Federal Identification Number

Step 3 – Notarization –

  • A notary will witness the signature
  • Notary shall complete the document with all required acknowledgement information

Step 4 – Inspection Sheet –

  • Included is an inspection sheet that must be completed before the sale of a vehicle. Provide this form when having the vehicle inspected at a licensed inspection station.
