Vermont General Financial Power of Attorney Form

The Vermont general financial power of attorney form is a legal document that will be authorized by the Principal to grant powers to a selected Agent who would oversee any finances pertaining to the Principal’s banking and other financial accounts, personal property and real property,in the event the Principal must be absent for an extended period, becomes ill or both. This document will not, however, grant any powers allowing this Agent to act on behalf of the Principal, to make any health care decisions.

This document should be thoroughly reviewed by the Principal, prior to completing the document. Should the Principal find the document to be unclear the Principal may wish to consider a consultation with an attorney for proper understanding and/or assistance with the document. Once completed, this document must be signed before a Notary Public and at least one witness. The document may be revoked any time, by the Principal.

How to Write

Step 1 – Titled Sections – Begin by reading:

Step 2 – The Parties – Enter:

  • The Principal’s name
  • Street Address
  • City
  • State
  • AND
  • The Agent(s) Names
  • Street Addresses
  • City
  • States
  • AND
  • The name of the Successor Agent
  • Street Address
  • City
  • State
  • Submit the state where the powers document shall be utilized by the Successor
  • Read all of the Terms and Conditions before proceeding with this document

Step 3 – Additional Titless – Review:

  • Third Parties
  • Compensation
  • Disclosure

Step 4 – Signatures – Must sign before a Notary – Submit

  • Print the Principal’s name
  • Date the document is being signed in dd/mm format
  • Enter the Principal’s Signature
  • AND
  • Print the Agent’s name
  • Agent’s Signature
  • Date the Agent’s signature in mm/dd/yyyy format
  • AND
  • Print the name of an optional Successor Agent
  • Provide the Successor Agent’s signature
  • Date signature in mm/dd/yyyy format

Step 5 – Notary Acknowledgement –

Once the Notary has recorded the ID information of the signatories and has witnessed all signatures, the Notary shall validate the document by completing all of the required notary information.

Step 6 – Acknowledgement and Acceptance Appointment as Agent and Successor Agent – Enter:

  • The Agent(s)must review the acceptance information
  • Enter the name of each Agent
  • Provide each Agent’s signature
  • Date signatures in mm/dd/yyyy format

Step 7 – Witness Attestation – The witnesses must read and agree to the attestation and enter:

  • Each witness must enter their printed name
  • Both witnesses must provide their signature
