Oklahoma Revocation Power of Attorney Form

The Oklahoma revocation power of attorney form is a legal document that will allow a Principal, who has provided a powers document whether it is a document to take over complete financial powers or a document to allow a Guardian to temporarily care for children, if a Principal is ready to revoke the powers granted to an Agent, this document will allow the Principal to remain in compliance with their powers document. Complete this document, sign it before a notary and have it delivered or served to the Agent(s). By state law, this document will revoke all powers to any Agent.

How to Write

Step 1 – The Parties – Enter the following:

  • The name of the Principal
  • The Principal’s Current Address
  • AND
  • Enter the name of the Agent(s) for which the Principal wishes to revoke powers
  • The Agent’s mailing address

Step 2 – Date and Signature –

  • Provide the date the revocation is being executed in dd/mm/yyyy format
  • Enter the Principal’s Signature
  • Printed name
  • Current address

Step 3 – Notarization –

As the Notary Public witnesses the Principal’s signature to this revocation document, the Notary will then enter the required information necessary to acknowledge the document by state law, with completing it by affixing the official seal
