Ohio Vehicle Power of Attorney Form

The Ohio vehicle power of attorney form is a legal document that will allow a Principal/Owner of a motor vehicle to assign an Agent to attend to the assignment or application for Certificate of Title for a vehicle as stated. The Agent is also permitted to perform all acts necessary needed.

This document must be signed in the presence of a state licensed Notary Public. This document may be revoked by the Principal at any time.

How to Write

Step 1 – Appointed Agent’s Information – Enter the following information:

  • Last name
  • First name
  • Middle initial
  • Street Address
  • City, State, Zip Code

Step 2 – Vehicle Information – Submit:

  • Make
  • Year
  • Serial Number

Step 3 – Principal’s Information –

  • Enter the date of the execution of the document
  • Provide the Principal’s Signature
  • Submit the Principal’s Social Security Number

Step 4 – Acknowledgement –

  • Once the notary has witnessed the signature of the Principal, the notary will then acknowledge the document and it’s contents and signature by providing the required notary information  and their official seal
