New Hampshire Limited (Special) Power of Attorney Form

The New Hampshire limited (special) power of attorney form is documentation of the authorization received by an Attorney-In-Fact/Agent to act on behalf of the Principal. An Agent will make decisions related to the Principal’s money, property, and various other items. All acts performed by the Agent shall be in compliance with the limited instructions described in the form, it is best to be as detailed as possible when relaying the instructions within the form as to avoid any possible confusion. To prepare for a situation in which the primary selection for the position of Agent becomes incapacitated, a secondary Agent can be selected and delegated within the limited power of attorney form.

How to Write

Step 1 – The Principal must download the document in PDF format  – the following titles should be reviewed carefully:

  • Revocation
  • Witness and Notary

Step 2 – Principal/Agent information – Next, the Principal must enter in the followin:

  • Name
  • Full Address
  • City
  • State
  • Agent’s Full Name
  • Agent’s Full Address
  • Agent’s City
  • State

Step 3 – If the Principal wishes to select a  Successor Agent, the following details pertaining to this individual must be supplied in the form:

  • Agent’s Name
  • Agent’s Full Address
  • Agent’s City
  • Agent’s State
  • Name of the State in which the document was executed

Step 4 – Before providing their Printed Name, the Date of signing, and their Signature, the Principal must review the following sections:

  • Terms and Conditions
  • Third Parties
  • Compensation
  • Disclosure

Step 5 – Page four (4) will demand the following information:

  • Agent’s Name
  • Agent’s  Signature
  • Date the Agent’s Signature in mm/dd/yyyy format
  • Successor Agent’s Name
  • Successor Agent’t Signature
  • Date the Successor Agent’s Signature in mm/dd/yyyy format

Step 6 – Notary Acknowledgement – To be completed by the Notary Public to attest that all signatures provided are legitimate and submitted with the Principal, Attorney-in-Fact, and Witnesses all in sound mind.

Step 7 – Acknowledgement and Acceptance of Appointment of Agent(s) –The Agent(s) must read over the document and acceptance paragraph before they supply the following:

  • Printed name of Agent and Successor Agent (if applicable)
  • Agent(s) signature(s)
  • Date the Agent(s) signature(s) in mm/dd/yyyy format
