The Minnesota revocation power of attorney form is a document that allows a Principal to provide written notice stating revocation of a previously applied powers document. By completing the document and delivering it to the Principal’s Agent, the Principal will immediately release the Agent from any and all previous responsibilities from the date of the document.
This document must be signed in the presence of a Notary Public
How to Write
Step 1 – Download the document –
- Enter the name of the Principal
- Enter the name of the Agent to whom the powers were granted
- Provide the date that the powers document was implemented
Step 2 – Signature –
- Read the statement
- Date the date of execution of the revocation document in mm/dd/yyyy format
- The Principal must enter their signature
Step 3 – Notarization –
As the Principal completes the document, the Notary shall witness their signature and complete the remainder of the document in acknowledgement of it’s validity
