Minor child power of attorney is a designation that is given by a parent who would like to select someone else to be the temporary guardian or their child for a temporary period of time. Most States restrict this type of power of attorney to a maximum time-period of 6 months or 1-year. At the end of the allotted time the parent may create a new power of attorney which would renew the designation for the time-period desired.

  • Initial and Check the box that best indicates the preference of the Parent/Court Appointed Guardian
  • Enter the state in which the document is executed
  • If limiting powers, list the powers in the lines provided

Step 4 – Date of Commencement – Enter:

  • The date in dd/m/yy format
  • Initial and Check one box that would indicate the preference of commencement of the document (A,B or C)
