Colorado Revocation Power of Attorney Form

The Colorado revocation power of attorney form is a legal document that allows a Principal who has created any powers of attorney, to revoke the documents. removing all powers from any or all Agent(s). The document must be completed, signed before a notary and delivered appropriately to all Agents and interested parties.

How to Write

Step 1 – Check the boxes that shall apply to the revocation –

  • Health Care Powers
  • Financial Powers
  • Other – Enter any other powers that the Agent may have access to that the Principal would like to revoke

Step 2 – Parties –

  • Enter the name of the Principal
  • Add any titled documents to be revoked
  • Enter the name of the original Agent
  • Enter the name stated as the alternate Agent
  • Enter the date the Principal would like the revocation to take effect
  • Print the Principal’s Name
  • Provide the Principal’s Signature (before a notary only)

Step 3 – Notary Acknowledgement –

As the form is completed, the Principal must provide identification and sign before a notary for the document to be effective. The notary will then complete the remainder of the form in acknowledgement

Copies of the document must be provided to any or all Agents with the Principal retaining the original for their records
