Alaska Revocation Power of Attorney Form

The Alaska revocation power of attorney form is form that is used by a Principal that has prepared any type or power document, that would like to revoke the document and advise the Agent(s) that their assistance is no longer required. This document will require notarization.

How to Write

Step 1 – Begin by Downloading the Document –

  • Check the appropriate box
  • If the document is something other than those listed, specify by checking “Other” and describe the power(s) or document that you would like to revoke

Step  2 – Revocation –

  • Enter the name of the Principal who is sending the revocation
  • Enter the title of the document
  • Enter the date in which the document went into effect
  • Provide the name(s) of any additional agent(s)

Step 3 – Principal’s Signature –

  • Date the document to take effect immediately upon the date provided
  • Enter the Printed name of the Principal
  • Enter the Principal’s Signature

Step 4 – Notary Acknowledgement –

  • After the form has been completed, the notary will witness the signature of the Principal and acknowledge the document by completing all of the required notary information an affixing the official seal.
